Progesterone Power: Understanding Why Delivery Method Matters

Progesterone is a superstar hormone that doesn’t always get the spotlight it deserves, especially when it comes to relieving hormone imbalance symptoms. Sadly, many healthcare providers overlook the crucial differences in how progesterone is delivered.

This can lead to mix-ups—like giving an oral dose topically (or the other way around)—and those mistakes can leave women frustrated when their symptoms don’t improve.

So, if you’ve tried progesterone and thought, “This isn’t working!”—it might not have been you, just the wrong dosage or delivery.

There are three main ways to use progesterone: oral, topical, and sublingual (under the tongue). Each has its perks, and choosing the right one depends on your specific needs.

Oral progesterone (pills) is the only form of progesterone available through a conventional pharmacy. Because of this, it’s the most likely form you’ll be given by your provider. It’s great at protecting the lining of  the uterus (endometrium) against any unwanted growth effects of estrogen, which is super important if you’re using a daily estradiol patch or gel. But there’s a catch – once you swallow it, your liver breaks it down and most of it becomes another hormone called allopregnanolone. While allopregnanolone helps with sleep and anxiety (yay!), it doesn’t leave much actual progesterone to tackle issues like mood swings or brain fog. Some women on oral progesterone feel “better,” but not totally great. In those cases, more pills aren’t necessarily the answer—too much can be rough on the liver or cause grogginess. Instead, pairing oral progesterone with a bit of topical progesterone might be just the trick.

Topical Progesterone: The Over-the-Counter Hero

Topical progesterone is easy to find—it’s at your grocery store or health food shop, and compounding pharmacies can whip up special blends just for you based on your provider’s prescription. The best part? It bypasses the liver, meaning what you absorb is all used as progesterone. No grogginess, but still a good boost for your sleep! It’s generally well-tolerated but doesn’t protect the uterus as well, so it’s not enough on its own for those using a daily estradiol patch.

Sublingual Progesterone: The Best of Both Worlds

Enter sublingual progesterone, where oral meets topical. This form, usually in a troche (a small, dissolvable wax), is placed under the tongue. Part of it is swallowed (offering endometrial protection), while the rest goes directly into the bloodstream, avoiding the liver altogether. It’s one of my favorite options because it balances the benefits of both oral and topical progesterone.
